Obtain a Choose Life Delaware License Plate

Instructions to become a member of Choose Life Delaware and obtain a State of Delaware "Choose Life" license plate are the following:

1. Complete the Choose Delaware Membership Form.

2. Complete the DMV form 613.

3. Make a copy of your current DMV Motor Registration Card.

4. Make your check payable to "Choose Life Delaware, Inc."

5. Mail the above 4 items to:
